Atria Mobile Service Terms & Conditions

  • By uploading laboratory documents, I authorize Atria Mobile to review my medical information

  • Once payment process is complete, patient party will be contacted for scheduling.

  • All appointments are scheduled within a 2 HOUR service window. (example: 8am-10am)

  • All reschedules MUST be made at least 1 hour BEFORE confirmed time window.

  • No refunds for service denials, abandoned appointments, or nobody home during confirmed visit time.

  • No refund for any cancelation or reschedule LESS than 1 hour before confirmed service time window.

  • To cancel or reschedule appointment call 888-404-9005

  • All refunds will incur a 10% chargeback fee.

  • Follow up visits for specimen pickups (urine, stool, etc.) may require additional charges.

  • Atria Mobile technicians DO NOT collect specimens from catheters or colostomy bags.

  • Atria Mobile technicians DO NOT collect wound cultures specimens or offer wound care

  • Atria Mobile is not responsible for any test or processing charges required by third party laboratories.

  • Atria Mobile is not responsible for any charges rejected or not covered by patient insurance.

  • Atria Mobile is not responsible for providing or retrieving test results from processing laboratory. 

  • Atria Mobile is not responsible for analyzing or discussing laboratory produced results with anyone
